How do I apply for a Birth, Death or Marriage Certificate?
Simply write the name and date of the record you are requesting and send it to our office along with the $5.00 check or money order made out to the Town of Warren , as well as, a self-addressed stamped envelope.
We will return the information the same day we receive it.
